 If you want to insure your horse for more than the purchase price, I’ll need a Justification of Value form submitted with the application.   

If horse is valued over 100K I also need a vet certificate.

If you’re a current customer requesting changes, I’ll need a Declaration of Health and a Justification of Value if you’re increasing a horse’s insured limit.
